The pilot episode began with two girls as they got hit by a train. Callie noticed the same tattoo on the arms of both girls and she discovered that the girls had a suicide pact because they wanted to be together. The girls survived the accident and were merely a topic of discussion when Jo arrived.

Jo and Alex had moved out of Meredith's house, which was now filled with people. Meredith and Amelia had been fighting over small things like the wall that they talked about taking down before. Amelia had taken a sledgehammer to the wall and the sisters had been fighting about it ever since.

One of the girls had a complication in the operating room, which panicked Meredith. Luckily, a candidate for the chief was watching and talked Meredith through the procedure. Bailey considered the position of chief and she made a compelling case for the board. She became the new chief.
