Cleveland Clinic to provide $2M for detransition care, won't perform sex-change procedures for minors

The Cleveland Clinic Foundation has agreed not to provide transgender-related surgeries and hormone drugs for minors and give $2 million to fund detransition care as part of a legal agreement.

The U.S. Department of Justice (DOJ) announced that the Cleveland Clinic entered into an agreement with federal authorities and Ohio Attorney General Dave Yost.

Under the agreement, the Cleveland Clinic will pay $308,000 to resolve allegations that it submitted false billings to public and private payers to secure insurance coverage for so-called gender transition procedures on minors. The facility has also committed $2 million to provide care for individuals who regret their gender transition, commonly known as detransitioners.

Civil Division Assistant Attorney General Brett Shumate said in the announcement that he was grateful for the clinic’s decision “to be part of the solution” and “not part of the problem.”

“Cleveland Clinic’s commitment to providing millions of dollars toward care for detransitioners is emblematic of just that,” stated Shumate.

“I am grateful for this resolution with Cleveland Clinic, but our work is far from over, and our division will continue to work tirelessly to protect America’s children and hold accountable those who have preyed on vulnerable children, whether they be pharmaceutical companies or medical providers.”

In July 2025, the DOJ issued a statement announcing that it sent more than 20 subpoenas to clinics and doctors allegedly involved in performing gender-related procedures to trans-identified children. The subpoenas were part of investigations involving alleged healthcare fraud, false statements and other matters.

Last month, the DOJ announced that Texas Children’s Hospital in Houston agreed to pay $10 million in damages for performing procedures on trans-identified children. The hospital also agreed to provide what authorities described as “the first-of-its-kind clinic dedicated to restorative care for detransitioners.”

According to the agreement, the Texas-based detransition clinic will be free of charge for the first five years.

Texas Attorney General Ken Paxton, whose office worked with the DOJ on the investigation, released a statement at the time calling the agreement “a monumental day in the fight to stop the radical transgender movement.”

“I applaud Texas Children’s Hospital for changing course and committing to being a part of the solution by agreeing to form a first-of-its-kind detransition clinic that will help provide free care to those who have been victimized by twisted, morally bankrupt transgender ideology,” said Paxton.

“Under my watch, I will investigate and bring the full force of the law against any Texas hospital that abuses children with harmful medical interventions to ‘transition’ kids.”

A district court judge ruled against the DOJ’s subpoena of Boston Children’s Hospital last September.

“The Administration has been explicit about its disapproval of the transgender community and its aim to end [gender-affirming care]. The subpoena reflects those goals, comprising overbroad requests for documents and information seemingly unrelated to investigating fraud or unlawful off-label promotion,” wrote Judge Myong Joun of the District of Massachusetts, a Biden appointee. 

“It is abundantly clear that the true purpose of issuing the subpoena is to interfere with the Commonwealth of Massachusetts’ right to protect [gender-affirming care] within its borders, to harass and intimidate BCH to stop providing such care, and to dissuade patients from seeking such care. ... The Government has failed to show proper purpose and, even if it had, BCH has demonstrated that the subpoena was issued for an improper purpose, motivated only by bad faith.”
